What is the File of Life?
The File of Life is a card that enables emergency medical personnel to obtain a quick medical history when a patient is unable to offer one. The card is kept in a magnetized red plastic pocket and is placed on the outside of the refrigerator.
The card lists, among other items, a person's name, emergency contacts, health problems, medications, allergies, recent surgeries and name of doctor(s).
The File of Life is an invaluable resource to emergency personnel responding to critical, life threatening situations.
In addition to the File of Life for the refrigerator, a compact version is also provided for a person to carry in a wallet of purse. |
How Do I get a File of Life?
Simply click on the Contact Page and view the list of local town/city offices where you can pick up your free File of Life. Then, just fill out the registration form and the File of Life information sheet.
